Why this happens
Wrong credentials fallback, OAuth ban denial mapped to this string, or unmapped login error. Prefer more specific messages when present (disabled, pending verify, captcha, rate limit).
Diagnose and fix
1. Recheck email/password; use forgot-password if needed.
2. Complete captcha if prompted; wait out rate limits.
3. Admin: check bans / account status for the member.
